antes do "end." a sua dll vai dar mensagem de erro ao ser carregada, mas o codigo nela será executado, porém, nenhuma thread sera criada caso isso ocorra, duvidas, leia o artigo sobre CreateThread no MSDN, nenhuma thread é criada no processo de inicialização de uma dll, o comando é dado mas só é efetivado depois que a dll é completamente carregada, o que não ocorre se o retorno dela nao for diferente de 0
